If you are injured on the job....

If you need emergency care, go to the nearest emergency room. Have a co-worker call an ambulance if necessary. Emergency care is covered under workman's comp at any emergency facility.

HOWEVER...

Any FOLLOW UP CARE must be completed at one of the four locations listed below.

 

OR...

 

If you are injured on the job and it is a minor injury that does NOT require emergency attention but DOES require the care or advice of a doctor, proceed DIRECTLY to one of the four listed locations.

 

If you go to a provider OTHER than one of the four listed below, YOU  will be responsible to pay for the care you receive! This is workman's comp LAW, not company policy.

If you are out of range of one of these providers, it will be better for you to go to an emergency room so as to avoid charges!
